What this document will cover
A Notice of Privacy Practices is a specific document required by law. When published, it will describe how your protected health information may be used and disclosed, and how you can get access to that information.
It will also set out your rights: to inspect and copy your record, to request corrections, to request restrictions on how information is shared, to receive an accounting of disclosures, and to be notified in the event of a breach.

You also have the right to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing one.
